12. Groin Hernia in the Elderly

12.1 Introduction

Groin hernias are amongst the most commonly performed surgical operations, with over 71,000 groin hernia repairs carried out in the UK in 2014/2015 [1]. About 600,000 groin hernia surgeries are performed in the United States yearly. Inguinal hernia classically presents maximally in early childhood, and then in the elderly, whereas femoral hernia does not exhibit this bimodal type of prevalence [2] (Fig. 12.2). Men have a 27–43% lifetime risk of groin hernia, whereas in women it is 3–6%. The only cure for inguinal hernia is surgery. The asymptomatic patients with groin hernias are a minority and even with watchful waiting 70% convert to surgery within 5 years. Though most groin hernias in adults are operated in the older age groups the various guidelines do not clearly address the issues of operating in this age group.
